- an applicant who was previously registered with the CANNN to practice as a Nurse Practitioner and who wishes to reinstate their registration, and
- an applicant who is currently registered as an Associate Non-Practicing Member with the CANNN and who wishes to obtain an active practicing membership as an NP (must have been registered as an NP with the CANNN in the past).
- Open your account on the Applicant Portal link (if your email has changed, please let us know so we can update)
- Once signed in, select Reinstatement application. You will be prompted to complete information like places of work, current registrations, etc
- If you have had a legal name change, please submit a legal name change form
- Pay processing fee – $65.63
Check your Member login on a regular basis to view what documents are required and/or outstanding
- Valid government issued photo ID
- Verification of registration from all jurisdictions with which you are currently or formerly registered.
- Form A1 Employer Reference is required from your most recent employer. Forms must be sent directly by the employer to CANNN
- Form A2 Evidence of Practice Hours 2023: For applications for registration for the remainder of the current year fill this form for hours going up to 2022.
- Form A2 Evidence of Practice Hours 2024: For applications of registration starting in 2024, this form is to be completed with hours going up to 2023 and is required from all employers in the past 5 years. Forms must be sent directly by the employer to CANNN.
- If you have less than five years work experience as an RN/NP and have had only one employer, please have Form B Colleague Reference Form submitted in addition to your employer reference form.
