Verification Request

• Do you need to verify the status of your registration at CANNN with another nursing regulatory jurisdiction?
• Do you need proof of your exam results sent to another nursing regulatory jurisdiction?
• Do you need proof of previous or current registration with CANNN?

Please complete the following forms and submit to CANNN if we are sending the information out to a school or nursing regulatory jurisdiction. Please ensure you include in the email where we are sending the verification.

2. Payment Authorization Form for all Transactions

Please note that if you need a verification from another jurisdiction sent to us please visit the jurisdictional website to complete their request form and payment.
